Upload the entire mailster-multi-smtp folder to the /wp-content/plugins/ directory
Activate the plugin through the ‘Plugins’ menu in WordPress
Go to Settings => Newsletter => Delivery and select the Multi SMTP tab
Enter your credentials
Send a testmail

This plugin would make a lot more sense if servers could be selected per campaign basis. Right now the server is switched every time you send a message.
